Chemonics Foundation Haiti (herein referred to as “Chemonics”), under the Global Health Supply Chain -Procurement and Supply Chain Management, USAID Contract No. AID-OAAA-I-15-0000, is issuing a Request for Proposals (RFP) for the Preventive Maintenance and Repair Services, for 9 Low (-15°C to -35°C) and 11 Ultra Low Freezers (-40°C to -86°C). The attached RFP contains all the necessary information for the potential bidder.

The offeror selected as a result of this RFP will be required to:
• Perform monthly maintenance in accordance with the instructions of the manufacturer.
• Provide cleaning, adjustment and verification of all systems as recommended by the manufacturer and in accordance with the characteristics of each piece of equipment.
• Inform GHSC-PSM one week prior to a visit to one of the sites for any specific intervention.
• Fix the freezer within (7) days after having received the written notification of the breakdown from the buyer.
• Prepare a monthly report that includes details of the brands, models, licenses, and corresponding serial number of all equipment for which maintenance has been done or repairs have been performed for payment purposes.
• Provide operational training to a maximum of three users per freezer who will be operating the equipment properly.
• Ensure that a field Service Engineer will be based in Port-au-Prince for immediate action.
• Provide all parts required to repair the equipment covered under its serviced obligation such as compressor, condenser, alternate plug, and air filter.
• Provide strategic recommendations for purchase of new low/ultra-low storage equipment.
