Introduction

Based in Port-au-Prince, this position is responsible for the implementation and oversight of the NASTAD Haiti Community Health Team (CHT) Project. Through this project, NASTAD and the Haitian Ministry of Health (MOH) will work with CHTs to focus and improve the quality of their activities related to HIV and patient tracking/follow up. The Coordinator will work with MoH and partners to revise and adapt practice tools for expansion of the project, which is currently in a pilot phase. The coordinator will also work with CHTs at the institution level to identify gaps, barriers and provide technical assistance.


Fonctions

System Implementation
• Coordinate and oversee the implementation of the system.
o Collaborate and ensure continuous buy-in from key MOH personnel at the central and departmental levels as well as site staff and network administrators.
o Provide training and supervision to staff and sites.
o Facilitate periodic CHT meetings at the local and departmental levels to discuss best practices, regional needs, and referral sources.
o Ensure the functionality of the system. Identify and communicate potential barriers, gaps and needs that should be addressed

System Monitoring and Evaluation
• Using the evaluation framework, track system progress and use and identify successes, needs and gaps.
• Work with the MOH and Key partners to assess the results of the pilot implementation.
• Based on pilot evaluation findings, introduce changes and improvements to the system as needed during the pilot process.

Outreach and Advocacy
• Develop and maintain strong, proactive relationships with key MSPP staff and others working in patient’s tracking system and community-based services.
• Prepare project progress reports and present the data.

Management
• Provide day-to-day supervision of the CHT System, including applicable NASTAD/Haiti staff and departmental contributors.
• Act as the main liaison to implementing partners to troubleshoot data transfer and content issues.


Qualifications Requises

Education/Experience
• Advanced Degree in Medicine/Nursing or Public Health preferred.
• Minimum of four (4) years of HIV or community health program experience.
• Strong experience in public health program planning and implementation, monitoring, evaluating, and documenting observed activities.
• Strong and demonstrated experience in training and group facilitation.
• Experience in data management and data analysis, with ability to perform simple reporting statistics.

Knowledge and Skills
• Excellent written/oral communication and presentation skills in French and Creole; English is an asset.
• Knowledge of HIV, including HIV and TB prevention, care and treatment.
• Ability to navigate complex government processes with multiple influencers, while negotiating and achieving consensus.
• Must be strong mentor, manager, role model and team player.
• Strong analytical skills and ability to write clear and accurate reports.
• Exceptional ability to work independently and autonomously.
• Ability to communicate effectively with partners, staff, and colleagues.
• Proficiency in Microsoft Office Suite with strong computer skills.

• Interest in working with an HIV/AIDS public health organization.
• Interest in working within a diverse work environment.
• Travel requirement may be up to 40% in and around Haiti
