Introduction

OVERVIEW: USAID and the World Bank are working together on a joint project with the Ministry of Public Health and Population (MSPP) to establish a contracting function in the MSPP to contract for essential health services. As part of USAID Forward, a USAID strategy to promote country ownership, and the World Bank’s policies in this regard, improvement of donor coordination, increased visibility of the MSPP over funds directed to the health sector, and increased efficiency of funding are all principles underlying this initiative.

The overall objective of this project is to achieve rapid health improvements through increased access to quality health services by strengthening health systems and building the capacity of the Haitian state to manage its funding sources and make functional health referral networks available to the Haitian population. LMG/Haiti will also establish functional health referral networks in the 12 health districts supported by the United States Government as part of its “development corridors” in Haiti.

To ensure implementation of the contracting of health services, which will include a system of performance-based financing (PBF) incentives to improve the availability and accessibility of quality services, the Unité de Contractualisation (UC) will be created within the MSPP. The UC will report directly to the Minister of Health in accordance with the newly-proposed Loi Organique of the MSPP. The employees of the UC will be hired and paid by the World Bank.

The UC will be at the heart of the contracting/PBF scheme which will also involve technical contributions from the different units and departments within the MSPP and from different administrative levels of the health pyramid. The implementation of this contracting apparatus will also catalyze the development of sectoral control functions in the ministry, and notable in the implementation of a programmatic approach and sectoral agreement between the Haitian state and its development partners.

The functions to be implemented as part of the UC are:
- Operations/Monitoring
- Administration/Finance
- Procurement
Each of the above functions will be managed by a coordinator. The UC will also include an auditor. The UC will be placed under the responsibility of a director.
The other departments and units of the MSPP implicated directly or indirectly in the implementation of the contracting and PBF functions are:
- La Direction de l’Administration et du Budget (DAB)
- La Direction de l’Organisation des Services de Santé (DOSS)
- La Direction des Ressources Humaines (DRH)
- L’Unité d’Appui à la Décentralisation Sanitaire (UADS)
- L’Unité Juridique (UJ)
- L’Unité de Planification et d’Evaluation (UPE)

Other units and departments (notably the directorates of vertical programs) will also be associated directly with the implementation of the contracting apparatus, providing, for example, definitions of health service packages, planning, coordination of care, etc.

It should be noted that the MSPP organogram is currently being reorganized and that the functions and services that make up these entities could be reassigned. Joint directorates or units would also be created, grouping several directorates or units together.

Decentralized administrative levels of the health pyramid (Directions Départementales Sanitaires (DDS) and future Unités d’Arrondissement de Santé (UAS)) will be closely associated with the implementation of the contract through data collection, verification, and validation systems, regional and local health planning, implementation, and monitoring by the health provisions included in the contracts. The UADS (or its equivalent in the new organizational structure of the MSPP) will be responsible for coordinating and supervising the work of these entities.

OVERALL RESPONSBILITIES: With LMG/Haiti, implemented by Management Sciences for Health (MSH), USAID is financing a capacity building program, particularly through capacity building technical assistance, in support of developing the necessary functions of the implementation of a health contracting unit and PBF by the MSPP.

We are seeking a Long-Term Technical Advisor (LTTA) to support the capacity building of the Unité de Contractualisation (UC). S/he will also contribute indirectly to the capacity building of other departments and technical units within the ministry in addition to ensuring the successful implementation and operability of the contracting/PBF scheme.

The LTTA will also, in coordination with the LMG/Haiti Senior Technical Advisor, provide technical assistance to certain directorates and units within the MSPP, supporting the activities of the Senior Technical Advisor and short-term and medium-term technical assistance providers recruited specifically to conduct activities at the unit and departmental levels. The LTTA will contribute to the coordination of technical assistance and identification of technical and training needs within the MSPP.

To accomplish these objectives, the LTTA will support the development of methodologies, tools, procedures and practices. This will be completed through sharing experiences and best practices among LMG/Haiti team members and MSPP staff, daily interaction to ensure the transfer of skills within the MSPP and also through the organization of thematic working groups, seminars, technical workshops and any other activity likely to develop staff skills within the units benefiting from this technical assistance.

The LTTA will work in collaboration with one or more counterparts in the MSPP units. These counterparts will be closely involved in activities lead by the LTTA or in which the LTTA participates. The LTTA will ensure active knowledge exchange with the MSPP counterparts in daily work and through sharing responsibilities in LTTA activities. Capacity building will also take place through trainings, technical programs, and workshops developed in response to needs identified at the beginning of and throughout the LMG/Haiti program.

The principal areas of intervention for the LTTA are:
• Contracting

• Budgeting and financial monitoring
• Communication


Fonctions

Technical assistance will be implemented through the following illustrative activities:
1. Review and update the LMG/Haiti technical assistance plan for the MSPP with the Project Director and the Senior Technical Advisor. This activity will include developing a timetable and logical framework for technical assistance provision (for long-term, medium-term, and short-term technical assistance) – including general and specific objectives, indicators to measure results, sources of verification, identifiable constraints and risks during this work, methodologies for assistance, operating procedures, available resources, a detailed activity plan and budget for the first project year.
2. In coordination with the Senior Technical Advisor and the Director of the UC, identify one or more counterparts within the identified MSPP units and directorates, particularly within the UC, to serve as a liaison during the implementation of activities.
3. Establish a Technical Working Group (TWG) on contracting and PBF with all stakeholders, led by a senior leader of the MSPP. The LTTA will support this TWG in developing the group's Scope of Work, identifying members, defining operating procedures, and assisting in budget development, etc.
4. Support the contracting and PBF TWG sessions by preparing agenda and notes for participants prior to working sessions, ensuring technical presentations are prepared for each meeting, sharing and distributing information on experiences in Haiti and other countries, providing TWG members with the documentation necessary to participate in their sessions, conducting debates, preparing and distributing meeting minutes and progress reports on TWG activities.
5. Evaluate the developmental capacity needs of the staff recruited for the UC, developing a training plan for UC members, and drafting scopes of work for short-term technical assistance to support the UC according to the previously identified needs.
6. Participate and contribute to other TWGs established in the MSPP as necessary to support the implementation of LMG/Haiti activities and ensure that the capacity to support the contracting/PBF scheme is developed.
7. Develop methodologies and tools identified through the TWG sessions.
8. Draft an information and communication strategy for the implementation of the contracting/PBF scheme with key stakeholders including public and private sector stakeholders and NGOs.
9. Prepare quarterly updates and working notes on the status of LTTA activities, detailing progress, and activities implemented, challenges encountered and results achieved. Participant lists from all activities should be included.
10. Collaborate with STTA and MTTA providers for the project working with MSPP units and directorates, ensuring the implication of MSPP counterparts in all activities.
11. Assist in annual workplanning and budgeting exercise for the LMG/Haiti Project.
12. Contribute to project reports in collaboration with the Senior Technical Advisor and Project Director as necessary.
13. Assist in the mid-term evaluation of the program and in the development of an exit strategy after 24 months of project implementation
14. Other tasks as required.


Qualifications Requises

1. Advanced degree (Masters, PhD, or MD) in a health-related field or management field required.
2. Ten or more years of experience as a technical assistance provider or manager within public, semi-public, or private institutions.
3. Demonstrated knowledge of contracting and PBF systems, preferably within a health context.
4. Experience with public health programs supported by bilateral agencies such as USAID and international agencies such as WHO, and World Bank, preferred.
5. Relevant global experience and previous experience in public health work in Haiti is beneficial.
6. Demonstrated ability to work in a changing environment.
7. Demonstrated strategic agility, diplomacy, conflict management, team building, written and oral communication and negotiation skills.
8. Excellent interpersonal communication skills; demonstrated ability to interact professionally with culturally diverse staff, clients, and consultants.
9. Demonstrated ability to assess priorities and manage a variety of activities in a time-sensitive environment and meet deadlines with attention to detail and quality.
10. Track record of, and strong commitment to, transparency and collaboration, as demonstrated by sharing knowledge, documenting experiences, supporting creative initiatives, and sharing credit.
11. Demonstrated ability to build and maintain relationships with senior-level colleagues, particularly interacting productively, proactively, and comfortably with government agencies, NGOs, private sector groups, USAID, Collaborating Agencies, and donor organizations.
12. Experience with monitoring and evaluation of program performance.
13. Experience managing and supervising technical staff.
14. Excellent language capability (written and spoken) in French (4 level) and English (3 level). Haitian Creole a plus.
