Introduction

The community Nurse will provide opportunities for staff involved in the care of patients with HIV/AIDS to update their knowledge and skills and improve decision making skills regarding patient care through community specific training and mentoring.


Fonctions

1. Provide education to community staff to increase their knowledge and skills related to patient care through community specific training.
2. Adapt educational material, teaching aids and community reference material to assist the point of service staff.
3. Monthly visit to each site and on as needed basis for needs assessment, mentoring community staff, and nursing and CHW Identifying challenges/problem solving including evaluation of current skills level. Participate in training program for nursing care, adherence, palliative care and support.
4. Develops and implements a professional development plan in conjunction with program lead
5. Participate in report writing, provide information, and act as a liaison and local partner treatment facility advocate.
6. Collaborate on all nursing issues and establish the best practice standards for community-based nursing practices.
7. Nursing duties, including assessment, coordination and case management of patients in a community setting.
8. Establish close collaboration between ALESIDA and community programs, to ensure continuity of care and services for the beneficiaries.
9. In collaboration with the ALESIDA Lead Nurse develop and implement community strategies that can improve performance
10. Provide mentoring and coaching to community health providers
11. Ensure rigorous monitoring of HIV key activities and indicators
12. Work closely with HIV providers to ensure that quality of service is delivered
13. Identify any barrier related to the management of services and provide possible solutions


Qualifications Requises

1. Higher Nursing degree: MPH (Community Nurse) preferred.
2. Teaching experience required
3. Current clinical practice as a community nurse
4. At least 3 years of recent experience in the care of patients affected by HIV/AIDS
5. Excellent French and creole language oral and written communication skills.
6. Capacity to work both in a team and independently.
7. The ability to utilize adult learning strategies in education.
8. Willingness and ability to travel extensively to work with the staff of the point of services
9. Ability to provide written communication in a timely manner and adjusted to various levels of comprehension.
