Catholic Medical Mission Board, Inc., (CMMB), the leading faith-based global healthcare NGO, is expanding operations in Haiti to support new program opportunities. CMMB is poised to add additional talented professionals to the existing staff in Port-Au-Prince. Currently, the organization seeks a Community nurse for its Project ALESIDA. The overall Community Nurse responsibility is to provide opportunities for staff involved in the care of patients with HIV/AIDS to update their knowledge and skills and improve decision making skills regarding patient care through community specific training and mentoring. The Community Nurse reports to the ALESIDA Clinical Coordinator.
Fonctions
1. Provide capacity building to community staff to increase their knowledge and skills related to patient care through community specific training
2. Provide monthly visit to each site and on as needed basis for needs assessment, mentoring community staff, nursing and CHW identifying challenges/problem solving including evaluation of current skill level.
3. Support the hospitals to reinforce or develop their community network in accordance of the ministry of health (MSPP) model to address broader heath care needs in the community
4. Develop jointly with the partner sites’ community outreach staff protocols and mechanisms for the tracking of lost to follow-up patients
5. Provide onsite coaching to partner sites community outreach staff on topics and issues related to patient retention in care, community distribution of ARV, tracking of lost to follow-up patients.
6. Work with the sites’ community nurses, social workers to put in place data collection and data reporting tools for community outreach activities
7. Provide mentorship and technical assistance to sites ‘social workers and community nurses to follow-up and retain 100% of ART patients enrolled in 12-month retention cohort
8. Develop and maintain a tool to report monthly on inactive patients and those returning to facility after tracking
9. Oversee the utilization of tablets by field agents and ensure that they all functioning and connected to internet
10. Interact with all the actors of the community outreach staff in the sites (social worker, community nurses, psychologists, community health workers) through face-to-face meeting and conference calls to be updated on any issues to quickly address them.
11. Challenge sites staff to identify community resources and opportunities to reinforce the impact of community interventions
12. Collaborate with the clinical team to track all patients in need of Viral load tests and design strategies suitable to achievement of viral load suppression among each ART patient
13. Design and implement strategies to track and link to care all HIV+ patients not enrolled on ARV as required by the Test and Start protocol
14. Write success stories, document best practices related to community outreach interventions and share them among all the partner sites to increase the overall performance of ALESIDA
15. Represent CMMB and the ALESIDA project in meetings with partner organizations
Qualifications Requises
1. Higher Nursing degree: MPH (Community Nurse) preferred.
2. Teaching or training experience required.
3. Current clinical practice as a community nurse
4. At least 3 years of recent experience in the care of patients affected by HIV/AIDS.
5. Excellent French and Creole language oral and written communication skills.
6. Capacity to work both in a team and independently and the ability to utilize adult learning strategies in education.
7. Willingness and ability to travel extensively to work with the staff of the Points of Service.
8. Ability to provide written communication in a timely manner and adjusted to various levels of comprehension.
